The undersigned certifies that the information provided is true and accurate and authorizes J-MAP America to investigate credit references and banking relationships.
Applicant agrees to pay all invoices according to approved payment terms. Invoices not paid within terms may be subject to a finance charge of 1.5% per month (18% annually) or the maximum rate permitted by law.
Applicant agrees to pay all transportation-related accessorial charges including but not limited to: lumper fees, detention charges, layover charges, re-delivery charges, driver assist charges, pallet fees, storage charges, warehouse handling charges, fuel surcharges, and any other approved accessorial services.
J-MAP America reserves the right to modify or revoke credit terms at any time. In the event collection efforts become necessary, applicant agrees to pay all reasonable collection costs, court costs, and attorney fees.
